Dr. Paul Davis is a nationally certified Medical Examiner (CRME) , and certified to administer DOT physicals. A DOT physical includes vision exam and hearing test. The physical will also check your vital signs.

To Prepare for Your Physical

You will be asked to fill out your portion of the California DOT exam forms at the time of your appointment. We will provide these forms for you at your appointment.

The cost of the physical is $80.00. We accept cash, check, Visa or MasterCard.
